Tezbox currently only allows you to view/manage one set of keys at a time. In your case either your fundraiser ICO keys or your ledger keys. Perhaps you can use your tezos-client with a public tezos-node? And if your current tezos-client doesn't work for some reason, you can recompile one (and you don't have to bootstrap the node). If you don't want to recompile one (or can't for some reason), perhaps you could use the docker image?

Completely my error here. The Tezos Wallet app needs to be open on the Ledger while trying to send funds. This was causing my problem. After I opened the Wallet app on the Ledger while trying to send funds, things worked fine.

It is most likely that your address is the root path - I recommend trying the import command without the derivation path. tezos-client did change a few months ago: it used to show 6 import command options - the first three were the 3 root paths (1 for each signing cuve) and 3 with a derivation path, i.e. /0'/0' I suspect you'll find your missing address ...
